Half-life: ~26 minutes
Tesamorelin is a synthetic analog of GHRH (full 44-amino acid sequence stabilized with trans-3-hexenoic acid modification) that is FDA-approved for HIV-associated lipodystrophy — specifically visceral fat accumulation. It reduces visceral adipose tissue with a favorable effect on metabolic markers.
Mechanism of Action
Binds GHRH receptors on pituitary somatotrophs, stimulating GH pulse release that in turn increases IGF-1. The increase in GH/IGF-1 specifically promotes lipolysis of visceral adipose tissue with less effect on subcutaneous fat compared to systemic fat-loss peptides.
FDA-approved 2010 (Egrifta) for HIV-associated lipodystrophy. Falutz et al. NEJM (2007, 2010) showed significant visceral fat reduction and metabolic improvements in RCTs.
GHRH + GHRP synergy — Tesamorelin/Ipamorelin is the recommended GH stack for visceral fat.
Both are GHRH analogs — note this is technically redundant but some protocols use Tesamorelin for 5 days/week and CJC on weekends.
AOD9604 adds β3-adrenergic fat mobilization independent of the GH axis — complementary for fat loss.
Both are GHRH analogs acting on the same pituitary receptor — typically do not combine.
Do not combine GHRH analogs.
Sermorelin is another GHRH analog — redundant.
