also known as: Mounjaro, Zepbound, LY3298176
Half-life: ~5 days
Tirzepatide is a dual GIP/GLP-1 receptor agonist approved by the FDA for Type 2 diabetes (Mounjaro) and obesity (Zepbound). It produces among the greatest weight loss seen in a pharmacological agent, with phase 3 trials showing 20–22% body weight reduction over 72 weeks.
Mechanism of Action
Acts as a "twincretin" — simultaneously activates GIP receptor (which improves insulin sensitivity and reduces glucagon) and GLP-1 receptor (which slows gastric emptying, reduces appetite, and increases satiety). The dual mechanism is more effective than GLP-1 alone.
SURMOUNT-1 trial: 20.9% mean body weight loss at 72 weeks with 15 mg dose vs 3.1% placebo (NEJM 2022). Superior to semaglutide in head-to-head SURPASS-CVOT comparison.
AOD9604 independently stimulates fat mobilization; may complement the appetite suppression of Tirzepatide.
GH peptides preserve/build lean mass during the significant weight loss from Tirzepatide.
MK-677 counters the muscle-loss risk of aggressive weight loss on Tirzepatide by raising GH/IGF-1.
Retatrutide is a triple GLP-1/GIP/glucagon agonist — redundant with Tirzepatide, do not combine.
Survodutide (GLP-1/glucagon agonist) targets overlapping receptors — avoid combining.
Mazdutide is a GLP-1/glucagon agonist — overlapping mechanisms, do not combine.
Cagrilintide (amylin analog) can be combined carefully but is often studied alongside semaglutide, not Tirzepatide — discuss with prescribing physician.
Use 5-Amino-1MQ between Tirzepatide cycles to maintain metabolic rate through NNMT inhibition.
Some combine a GLP-1 agonist with Tesofensine for enhanced appetite suppression, though side effect monitoring is required.
Storage
Refrigerate at 2–8°C (36–46°F). Do not freeze. Can be kept at room temp for up to 21 days.
Reconstitution
Pre-filled pens (pharmaceutical). Compounded requires reconstitution per compounding pharmacy instructions.
